Just built a pole barn with 14 foot overhead doors. Minimum clearance for backing in most larger fivers..IMHO

Yes make it wide enough to put the slides out and the awning out . So things can dry out if wet. No more musty smell from a wet awning rolled up and stored that way.

Always go 4 feet longer than you think. Right now the travel trailer at the father-in-laws has the door closed down on the hitch. He got a bigger travel trailer than what he 'ever' thought he would.

Semi trucks are normally 13.6 14 being the limit before a permit is needed... So My vote is 14 foot door clearance when open.

In terms of width.. I would go full open, sliders, awning ETC. and if your slide is short figure more space...

Length well... 35+ foot + truck length + a few..

Now what do I have .. just a dream of a garage. so I sit in envoy......

One thing that I would add is in floor heating, even if you dont hook it up for awhile its hard to install it after the fact... and really cheap pipping wise during your pour.
