Axle flip and 4" sub frame added under fifth wheel and I don't have enough clearance yet but it works.

Hummm... I must have missed the OP's point!
Here's what I'm thinking.... Isn't your pin box adjustable? Mine is a Mohr-Ryde and the pin can be adjusted to 3 different heights. Wouldn't that solve the problem?
Continuing to make myself a fool... if your pin box is NOT adjustable, I think I would opt for a new pin box before flipping axles!

Sorry if I am way off base...
In my situation, if I would have just adjusted the pin box I would have been either very nose high or wouldn't have had any clearance between the FW and the truck bed. That's why I fully extended the pin box to get the clearance and then added the 4" trailer lift to level the FW.

You can replace your 3 step RV steps with a 4 step model. Some of the new fivers are coming with them. Or you can build a little box to set at the bottom if raising the fiver has pulled the bottom step too far from the ground.

Change to a standard pin box-it usually works. Gets the pin box inside the rails. In looking at some new 5vers it appears the pin boxes and mounting wings on some models are being cut on an angle to help with higher bed rails.

When I went from my F250 to my Chevy 3500, I had to drop the pin on my 5er. Even though the truck was lower (F250 had the offroad package) the Chevy had a deeper bed.
Many factors play into this beyond the height of your truck.

I had same problem. I flipped axles and still hit my trailer when going down with front of truck and trailer coming up a hill an inverted V situation. I had a body shop cut from the center pin pockets on bed of truck down and the toward the top of the taillight and cut the tailgate down also. It was done to a 2004 Dodge 2500 4x4. The cost was 1500 dollars but well worth it to me. I no longer have to worry about going over a bump or rise. I do do a lot of offroad camping with a 30 WBLE Cardinal.

It seems as though the 5er manufacturers are getting smarter. My new 2012 Cougar came flipped from the factory and it rides pretty close to normal. It also has mor ryde equalizers from the factory and the pinbox is chamfered at the back to keep from hitting the side of the box. I may need to do a little tweaking but it was pretty good right out of the box when hooked to the 3500HD DRW.
