I have been around the “Out-of-Country” travel insurance game for 6 years now. I am not an expert but I have met hundreds and hundreds of travelers that THOUGH they had coverage. But at the time they needed it most they found out they didn’t have what they thought they paid for. As a Canadian travelling to the US the strongest advice I can give you is;
Go through a Travel Insurance Broker,
Be blatantly honest with every question on the application,
Have your Doctor fill out the medical questionnaire with you.
Make ABSOLUTLY POSATIVE your insurance is “Underwritten at the time of sale not underwritten upon claim.

OP here - The primary reason for my concern is due to hearing horror stories about folks who "thought" they were insured, but later got huge bills for medical treatment while away from home. So I guess I'm looking for a provider that has a good reputation for honesty. The following link is to a CBC Marketplace video that investigates Manulife, and others, for their tricky wording in the medical questionnaires. Seems the questionnaire is written by company lawyers, and there's no way you can accurately answer it.
Note... this video is 22 minutes long, but very informative.

I do not think it is a question of 'honesty', just interpretation of the rules. I do not think any company would be less or more likely to pay a claim on a debatable point. If they possibly can get away with paying it, they will.
As far as Visa travel insurance goes, just be aware that the 31 days, as in the case of RBC insurance, begins the day you leave your home province and NOT when you leave Canada. We will drive back east to visit family and we have to be careful then if we go to the US to do some shopping.
With regard to Americans in Canada, why wouldn't they get travel insurance like we do? I know that if they are caught up here without insurance, then a clinic is the best bet rather than an ER, who will charge the full scale rate, insurance or not! I also believe they will treat you, evidence of insurance or not, if it is a life threatening condition.

You are right on the money Jag. That is one of the reasons that I indicated your policy must be Underwitten at the time of signing not at time of claim.

We use DAN which provides divers with insurance coverage at very reasonable rates and has a 24 hour hotline to call for help with a medical problem. They cover non-diving accidents as well and the policy includes air evac costs. Often where you are injured is not where competent treatment is available.

For A US citizens going to Canada or any other foreign country that is covered by Medicare there is no coverage (unless going to another US point). The Medicare Supplements may provide coverage out of the country up to $250K with a deductible amount. There is no foreign coverage on Medicare Supp. A, B, K, L.but the rest will have the foreign coverage. A Medicare Advantage Plan may not have foreign coverage or even out of state, depending on the plan. You would be out of the network for many of them. All Medicare supplements with the same Letter plah are the same with the only difference being rates and service.
