I bought a book for maintenance from CW. Works great for me. I carry it with me in the rig too.

I designed my own using Open Office (clone of Microsoft Works). Pretty simple. I keep records of all maintenance, oil changes, etcs. In addition, I log information about the various camps where we stay. If you (or anyone else) wants to take a look at it, PM me with an email address and I'll send the file. You can tailor it to your own needs.
This record came in really handy recently when I sold my twelve-year-old gasser. I was able to show the buyer complete records including gas mileage for my logged 69,000 miles.
Open Office is free and downloadable from the 'Net.
