I always slide my truck under the hitch with the handle in, that way I can see the handle pop out/in. After that I check that the jaws are wrapped around the pin. Reese 16k slider bought with the trailer, second truck.

Check to make sure the kingpin goes below the Jaws. It may be the jaws are not closing fully because the Kingpin is larger at the bottom than the middle. My reese will allow the relese pin to lock like this and still allow the kingpin to slide out. I usually close the jaws, drop the trailer all the way, then put truck in "N" to let it sellte, then check everything a second time.

I have a 22k Reese hitch, this is how I hook up. I always make sure the trailer is a bit lower than the fifth wheel so when I back up the trailer actually lifts a bit onto the fifth wheel and I can see the handle move out then in. I get out of the truck to make sure the jaws are wrapped around the king pin. Sometimes I do a pull test to make sure it's locked but not always & so far knock on wood I haven't had any issues.

I'm not sure what the life expectancy is but its 2 years old maybe 6,000 miles on it. I would think they last longer than that.

I actually just read that in the owners manual about having trailer lower and "forcing" it (plate) under. I've never done that but will from now on. I also wasnt aware you could have the jaws closed - handle in, when you hoked it up. I will also look into that.

I dont believe I have a "sfety pin" there is a little flapper that drops down into a groove on the handle so you can put a padlock on it. Is that what youre refering to? If so - no that was not down and locked in place. I am using a teflon plate, and will check that out. I'm really starting to lean towards operator error, as I probably didn't check things out as well just pulling out of the shed like I would have if going on the road.

you are doing your pull test with the landing gear too high. I would try it with them in a lower position

I think you are raising your TH to high when hooking up. I always force my TV under the TH when hooking up. Then get out and check the connecting.

I too always have the rig about 1/4" lower than the TV, where it has to do some minimal lifting (at idle speed). I also never move it until my safety caribiner is inserted and has the handle locked in place.
