There are small low cost TT like the Jayco swift, KZ and a few others that have a 14' TT. They are less then 10K new and light enogh to be pulled by your van. They will have a small kicthen and bath. They will also be able to be sold in the future. What your wanting will have very little resell value. You can take out what you dont want and store it with hardware to be put back in for resell. To have a custom unit built will cost a lot more and less resell value.

Dalton RV in North Ga. carries a line of small travel trailers called RWTraveler. They listed 13'-18' trailers. I've seen a 13' one with only a booth dinette and a toilet/tub bathroom. or a 15' one with just the bathroom.
They only have a 18' one listed on their site right now, but you may contact them and find out about the product line.

We tried very hard to find someone who would build us a custom little unit to our specs -- Carson Trailer in Gardena, Calif. came close to sealing the deal and then they dropped the ball and would not return phone calls. (Maybe they found me annoying?!?) So we bought a little Fun Finder and modified it extensively -- see my blog link below. I would be very wary of towing it with a minivan -- the brakes might not be beefy enough for the Sierra.

I would call them as a smaller manufacture, from all I heard when researching them, they were very "Customized" Friendly.

Take a look at what Taylor Coach has to offer. The company is based in Southern Ontario and Brad and his family have been in the trailer building business for many years.They build custom light weight TT's and will work with each customer to incorporate their own "wish list". We previously owned a 12ft model from them and now have a larger 20ft on order. We were at their business a few weeks ago and saw a small 12ft that they had just completed for a customer that only had in it 2 single beds and large closet and night stand.

I probably wouldn't use a sink or stove inside the trailer anyways as I'm quite happy to cook and do all the cleanup outside. Even a fridge could be replaced with a higher-end electric cooler (or two) like an Engel or similar.

Another vote for the converted cargo trailer here. That route is the correct one for many people, and I, for one, respect your inquiry. Converted cargo trailers have their own forum page, http://www.tnttt.com/viewforum.php?f=42.
An added bonus is that many cargo trailers come in narrower widths, say 5 foot, that would be a piece of cake for any minvan because of substancially reduced wind resistance caused by less frontal area. The lower wind resistance is better than most imagine, and some folks on RV.NET have commented on how they tow much easier than their own common full height/width TT. In these days of waning fuel economy, it is refreshing to see someone seeking more satisfaction by towing less.
For the disabled, entry is often improved by drop-down rear ramp access.
The unmodified cargo trailer is also a stealth camper in that it is less likely to be questioned as illegal camping while parked on any street overnight.

I was looking for pretty much what the OP had listed. I wanted to park it in my daughter's yard for when visiting. I would be eating, using the bathroom etc. in her house, but as there are only two bedrooms being used by DD, DSIL, DGK'S, it would have been a step up from sleeping on the couch, or setting up a tent in the yard.
