Drive with the OD off is not putting the shifter in 3. There is no 3 position on the shifter on your E350. The later transmissions have 1, 2, 3, and OD on the shifter. Yours has 1, 2, OD.

That's not how a Ford works. I was a Ford trans engineer from '88-'07. We programmed transmissions to stay in 2 when the shifter was in 2, and the same for 3. They will not start in 1st gear when the shifter is in 2 or 3.
This is true. In fact. I have often started out in 2 in slippery conditions. Starting in 2 helps to keep the rear wheel from spinning. In Fact. Fords manuel tell you to do it that way. A Ford. Will stay in the gear you tell it too. Good Ford, Good Ford.
Our Jeep on the other hand. Would start in 1st and go to 2nd, If I put it in 2. Didn't like that.
Interesting, as the Dodge autos don't do this. You put it in 2nd, and the tranny has access to both 1st and 2nd. Both my Acclaim and Dakota followed this setup. Putting it in 2nd only indicated that the tranny was not allowed to shift higher than second (however, see below). I also found that the tranny, if shifted into 2nd, and was in 2nd, might lock up the TC, as the RPMs would change slightly, but I can't be sure.
Having said all that, there is one situation where an auto tranny WILL disregard a shift selector. And that is if the engine is about to over-rev. Then the tranny will upshift to the next gear. However, once the engine speed has come down enough to safely downshift to the selected gear, it will do so. This is to protect the engine. I'm not sure if it shifts when coasting or if only under power, but it will shift. My Acclaim (1992) would do this, and so did my Dakota (2001). My current Ram (2004) is a stick shift, so it stays in the gear I select - this means I can find the governor (at around 5700 RPM) on the engine.

Nope. With the Ford. Put it in 1 or 2, and it will stay there. No matter how hard you press the gas. It won't shift till you want it to. Really sounds good with glass packs. Especially when you let off, and let it back down.
However. If you are in drive, and pull it to 2. It won't down shift untill it reaches a certain speed. Unless you floor it. Then it will down shift, and stay there.
Why would anyone want it any other way? I have come down many a mountain in 1 or 2. It holds back the camper great, and I hardly ever have to hit the brakes.
My van will shift into second if I manually have it in first.
It will take off, whine high, but then will shift to 2nd gear.

Hmmm. I have a 6 Spd Allison. The times I have used the "M" mode I have found it will not upshift, but will in fact down shift.
I had thought at first that by using Manual mode it would be easier going donwhill or on curvey mountainous roads. Well that just seemed to increase my tranny temps to the point where I started to get worried. I later learned that using tow/haul was the way to go. Seems that mode will lock the torque converter in at 2nd gear, leading to lower trans temps and much better control donwhill and other types of driving.
So although the 6 Sp manual is a neat kind of thing...I don't use it much at all. (not sure how/if the racers use it..??).

Have you tried over-reving the engine? It should upshift to protect the engine, but will downshift as soon as it's safe. I find it odd it won't downshift if you pull the selector. My Dodges would immediately. Make quite the jolt sometimes, unless I could add loud pedal just at the right time to speed the engine up.
The only problem with putting the selector in 2nd and having the tranny STAY in that gear is it makes it very difficult to get going from a standing start. And in my Dodges' cases, if you were starting in 2nd, it meant the tranny was in limp mode, and it was time to find a dealer/code reader.
I do agree with the hold back by downshifting. In fact, my Dakota's cruise control would downshift the tranny from OD to 3rd if the speed climbed more than 5kph above the set speed. It made for easier driving on some of our Island highways that are fairly hilly.

Yup, that's the TCM and ECM protecting your engine. They're seeing the engine speed climbing to the redline, so the ECM is commanding the TCM to override the selector and upshift to protect the engine. Once the speeds come down enough, then the TCM will be commanded to follow the the shift selector, and it will downshift back to the selected gear.

yea try going down hill and push it past red line, it will upshift, before it will grenade the motor/tranny.

My Titans 5 speed will allow me to downshift to any gear(except if knows it will over-rev)and hold it there. If I manually shift from "D"(5th) to 3rd the truck will still start off in 1st and upshift until it gets to 3rd, but no higher. Going down hills I can manually downshift to keep vehicle speed down and have done so...

I don't like autos that you can't control and I don't like autos that you can't feel shift, I do like the C6 in my truck.
If you select a gear it should start and stay in that gear, period.
