Rand McNally Road Carriers Atlas - you can get it at any truck stop.
Good large maps with all of the truck routes marked and a list of low bridges by state and highway number. Gives an accurate description of exactly where they are.
I carry one in the cab all of the time.

Boy I wish I was 12'1". At 13'6" you have a real appreciation for low bridges. I think the next book I buy will be the Rand McNally Road Carriers Atlas.

Not all, but many. Same as the other directories and web sites mentioned so far. They have lists of low bridges on bigger roads, but they most certainly do not have all of them. The best you can hope for is most of them on the roads that are likely to be travelled by big rig trucks. Side roads and lesser used highways will generally not have their low bridges listed.
I don't mean to belittle any of the suggestions made so far, they are all good and useful. Just don't put 100% of your faith into them.
