I too level the kitchen counter. I've checked it against the floor, the frame and the bumper and they are all pretty close. If the counter isn't level, it seems to be the most noticeable. The refrigerator is a non-issue. It has a range of a few degrees wherein it is fully efficient. Again, if the counter is level the fridge is close enough.
One thing I have noticed is that if you are on soft ground, what starts out level won't be after a couple of days. It tends to rotate toward the slide side. Using long 2x10's under the wheels on both sides seems to alleviate that problem the most spreads the weight out over the most area).

I agree that you should use a level inside the coach, either on the floor or counter top, but once you get it level once you can put a stick on level on the front by the landing gear switch. Just make sure the bubble is at zero when you stick it on.
I've got 3 slides on one side and have always had trouble getting it to stay level once you put them out. I think next time I'm going to try extending the slides first and leveling side to side before I unhitch from the truck. Any reason I shouldn't do this?

Yes, there is. You can really screw up your slides if you try to put them out when your RV is not level. Think of a cork screw, and then think of your slide out. Want your slide out to end up like one?

If by leveling, you would be driving or backing onto blocks, then yes, this is a bad idea. You would put an awful lot of stress on the slide out mechanisms if you move the trailer with them out. Especially if you have to drive it up onto an inch or two of blocks.
