Update so far
My dentist made a guard for me (actually two different thickness's) .
I'm working very hard during the day not to clench my teeth (also sometimes when I'm here ).
Since I'm a mouth breather, it doesn't appear I'm doing it at night.
The loud, bone breaking sounds, when I chew have decreased to a manageable level and they are gone now when I talk.
No pain and I have had some very minor headaches which I don't know yet if they are tied into the TMJ although I know it can cause migraines.

After breaking and cracking countless teeth I was told I had to wear a mouthpiece at night and now I can't sleep without it. I wake up so much more rested, snore less, no headaches and earaches. I've had the same device for 8 years and I take it each time I go for a cleaning and make sure the fit is still appropriate for my bite.

I guess I'm lucky - I was diagnosed with TMJ about 10 years ago; dentist sent me to a specialist, he suggested I wait a bit before starting any treatment & after about 4 - 6 months the clicking went away. Only time I have any problem is when I'm having my teeth cleaned - When I'm holding my jaw open while they scrape & poke I can feel it...
