On a 2004 Cardinal 5th wheel,how do you adjust the hydraulic cylinder shaft on the slide outs?On the end of the shaft the nuts are not tight on the bracket.

That is "slack" and is there for a reason....unless the slide is not opening or closing(sealing) properly do not adjust those nuts!! I have seen the situation and asked the same question..I am repeating what I was told....get a qualified person to give you the answer and to the adjustment if neeeded...My own experience was,, finding the nuts on cylinder end loose.....that side has to be tight...

The slide in the bedroom does not go in straight.It moves from side to side.

I am also interested in how to adjust slides. I have a 2006 Gulf Steam "Prarie Schooner" 29FWB. I have tried to get dealership to adjust and get them to close evenly. Thier answer was to spray silcone lubricant on the seals to no avail. This was done in the 1 year warranty period. So now I am having to do this myself.
Both slides are off about an inch of closing all the way and looke to be closed askewed. They don't leak and maybe should leave well enough alone. Any suggestions?

If your slide does not go in and out parallel to the side of the rig, you need to check your gears that keep both sides of your slideout moving together.
Hydraulic slides requires this as they have no other way to move both sides equally. The hydraulic cylinder/pump only provides the horsepower to do the work. The long bar with gears on each end keep it parallel.
