I stood up on the floor and told everyone about my infraction, I then told the boss that in the future if I screwed up to man up and come to me and speak to me face to face. The work place is no place for kindegarten behavior. Everyone is an adult and should be able to face their responsibilities like one.
Of course managment kind of treated me with kid gloves anyway. It sometimes helps if you are considered to be a bit unbalanced.
So what was the infraction that you as an adult wouldnt have known about in advance?
The infraction was nothing that I didn't know about,as a matter of fact it was a routine thing that we did twice a day.
We were required to log vehicle mileage at the start of the tour and at the end. It was a busy day and I forgot to record starting mileage, thereby "Falsifying government documents". Just a simple record keeping error, I discovered the mistake myself brought it to the bosses attention so a manual correction could be made.
Some bean counter at HQ apparently considered this to be important enough that it was worth 70 some people losing 15 minutes of production each to warn us of the dire consequences that would be brought to bear on the next unlucky person that had a momentary lapse.
Would have been so much easier if the boss had just come to the guilty party and said,"Try to be a bit more careful in the future,OK?"
In my company, we just had to issue new employee dress code letters that included safety gear (electricians). It is stated in our employee handbook several times what they are to wear on all jobs, but we still have a couple of employees who think that because they are not seen by the owner regularly, they do not have to follow rules. The offending employees were spoken to.
A day later, the superintendent went to another job and found a couple of other employees who were not wearing their safety equip. He was asked "Bob" doesn't wear safety glasses, why do I have to?".
Blanket warning to all employees - you will all wear safety equipment, no excuses.

Are you the only person that ever forgot to log that? Could be you were one of several that did it recently, and the bean counter or maybe even the boss thought it might be beneficial to do blanket reminder. Could be they thought hey, he is so thoughtful and never forgets things like that, maybe the rest of these maroons need a reminder before they screw it up.
I would e-mail back and ask if I was guilty of the offense referenced in the reprimand and if not, why was I being reprimanded for something I did not do.
Dear Boss,
I have received your reprimand memo in error as I have never been late for work nor left early without permission. If you have evidence to the contrary, I would appreciate a private meeting to discuss same rather than a public reprimand over an offense I did not commit.
