Can anyone recommend a good product for a dually inner wheel valve stem extender? What are the downsides of using any of these, if any? Thanks!

I’ve never owned a dually personally. However, I have been responsible for a number of them on fire engines and ambulances. They are very prone to leaking, and we have some quality ones. My recommendation would be a good air chuck with a flexible hose and angled valve stem turned to be able to connect it. I’m sure others will have their opinions, but that’s mine based on experience. I don't use extenders,,Valve cores come in many sizes, have your tire dealer install one that will extend far enough to air the tire .

Best to just have Borg solid valve stems installed. Extenders leak.

The valve stem extenders that came on my motorhome leaked and the inner tires went flat on me. I took the extenders off and put a TPMS system on. No more flat tires since doing that 6 years ago.

Our TPMS Leaked so we removed it.

Have had 3 duallies.......
Not sure what valve stem extenders are, but rumour has it, they are WAAAAAAAAAAY over-rated.
As noted, get a chuck that fits your rim openings, same with the gauge, and you will be fine.
I personally prefer the gauges and chucks with the straight end, and an angle for the outer dual.

My experience has been positive for 20+ years, various MH’s. Properly installed, quality made extenders should not leak. Mine haven’t. Mine are steel mesh ones, don’t know who made them. I do have my tires inspected every year.
A one piece stem makes a lot of sense though.

….. Can’t answer the question BUT, …. I got a Jaco Lightning T series tire inflator with a swivel head and airing up the tires went from Stupid Impossible to pretty darn easy …. I also have a TPMS….. On Amazon and worth every penny!!!

Owned a RAM Dually since 2012. I have the stock valve stems and see no reason to add extenders!!!
Use the proper double ended air chuck and life is GOLDEN.
